H-5 2/H-5 1 matrices. LA press. Mfd. by Apple Records, Inc on Side B label. Upside-down back slick. Light bubbling to front slick, bumped corners, light creases. One or two light sleeve scuffs. Nice copy of this.

The text arrangement on the labels is different from
other similar versions. 

Some of the tracks have been combined into medleys
with side 1 having five numbered tracks
and side 2 having seven numbered tracks. 
On later versions there may be more than 12 total numbered tracks.

Includes an insert with album credits and a photo on it.     

Errors: There are two known printing errors that exist for this version (LA)
The first one is a typo error with a "3T" prefix printed on the spine, instead of "ST"

The second error resulted from the raw card sleeves being
improperly oriented when they were loaded onto the machine
which is responsible for applying the graphic slicks to the card sleeves.
This resulted in sleeves which had the disc opening on the left side of the sleeve
instead of the standard right side..
